How the Referral Process Works

 

Step 1: Submit a Referral

Complete our online referral form, call or text (970) 829-8780, or email info@momentumcac.com.

 

Step 2: We Contact the Patient

Our Client Care team reaches out to the patient within minutes to discuss their needs, verify insurance, get them matched with a provider, and answer any other questions. 

 

Step 3: Patient is Matched and Scheduled 

We match the patient with a provider based on their specific concerns, preferences, and availability, and schedule their first appointment.

 

Step 4: Treatment Begins

The patient begins therapy, psychiatric services, or both. With a proper ROI in place, we can communicate with your office as needed to support coordinated care.

How Do I refer a patient to Momentum? 

With a proper authorization, a medical provider can refer a patient by completing our online referral form, calling or texting (970) 829-8780, or emailing info@momentumcac.com. Our intake team will reach out to the patient and get them scheduled with a provider who can meet their unique needs. Patients can also seek services directly. No referral is needed to get started.

Will I receive updates on my patients care?

With a proper ROI in place, we can coordinate with your office and share relevant treatment updates. As long as the patient wishes us to, we are more than happy to coordinate care. 

Does the patient need a formal referral or prior authorization? 

No. Patients do not need a formal referral from their PCP to receive therapy or psychiatric care at Momentum. Our Client Care team will verify coverage and handle any insurance coordination during the intake process.

What insurance plans are in network for therapy and psychiatric care?

We work with a wide variety of insurance plans, including medicaid and medicare. Our intake team can verify the patients coverage when they reach out.

Are sessions available online? 

Yes. We offer secure, HIPAA compliant, telehealth appointments anywhere in Colorado. We also offer in-person sessions at any of our office locations

What ages do you work with? 

We provide therapy and psychiatric services for children, teens, young adults, and adults. 

Can I refer a patient for both therapy and psychiatric services?

Yes. We offer an integrated care model where therapy and psychiatric services are coordinated under one practice. Whether a patient is seeking therapy, psychiatric care, or both, we will get them matched with the right provider. 

Do you offer family therapy? 

Yes. Our team offers family and couples therapy as well as individual counseling.
